AI-assisted risk stratification
Home-collected signals (BP, weight, symptoms, point-of-care or home eGFR and haematology) feed a risk model that predicts decompensation and CKD progression — surfaced as prioritised tasks, not alerts.
Clinician escalation layer
A qualified physician is accountable for every escalation. Algorithms suggest; clinicians decide. Every action is captured for audit and clinical safety.
